What Is a Short Menstrual Cycle?

A normal menstrual cycle ranges from 21 to 35 days. Cycles are measured from the first day of one period to the first day of the next.

Cycles consistently shorter than 21 days are considered short. Additionally, if your cycles have become significantly shorter than they used to be (for example, dropping from 28 days to 23 days), this change is worth noting, even if still technically within the normal range.

Some women naturally have cycles at the shorter end of normal and this is perfectly healthy for them. The concern arises when short cycles are associated with fertility difficulties or represent a change from your usual pattern.

The Two Phases of Your Cycle

Follicular Phase

From day 1 of your period until ovulation

Luteal Phase

After ovulation, the empty follicle produces progesterone to prepare the uterine lining for potential pregnancy. This phase is typically 12-14 days.

Short cycles can result from a shortened follicular phase, a shortened luteal phase, or both. Identifying which phase is affected helps guide treatment.

How Short Cycles Can Affect Fertility

Short Follicular Phase

A shortened follicular phase may mean eggs have less time to develop fully before ovulation. This can be associated with declining ovarian reserve (fewer eggs remaining), as the body may recruit follicles faster. It may affect egg quality, though this isn't always the case.

Short Luteal Phase (Luteal Phase Defect)

A luteal phase shorter than 10 days may not allow enough time for an embryo to implant, or may indicate insufficient progesterone to maintain the uterine lining. This can contribute to difficulty conceiving or early pregnancy loss.

Predicting Ovulation

What Causes Short Menstrual Cycles?

  • Declining ovarian reserve, As egg numbers decrease (often with age), FSH levels rise, leading to faster follicle recruitment and shorter cycles
  • Perimenopause, The transition to menopause often begins with shorter cycles before they become irregular or longer

  • High prolactin, Elevated prolactin levels can disrupt normal cycle patterns
  • Stress and lifestyle factors, Significant stress, extreme exercise, or major weight changes can affect cycle length
  • Natural variation, Some women simply have naturally shorter cycles that are healthy for them

  • Your cycles are consistently shorter than 21 days

Investigation and Treatment
